Gilt-stamped purple cloth over boards, illustrated with several black and white images of Oakwood Cemetery, has two pages of blank forms for someone to declare their wish to be cremated as well as two pages of blank affidavits to accompany a body to the crematorium, one form missing from each set of pages.
